Eurofins BioDiagnostics Plant Health Services provides Custom Disease Testing and Diagnostics - disease detection of plant pathogens. Please note that it is best to submit samples to the lab early in the week to preserve the integrity of the sample and accurate processing. Due to local regulations, please label the box containing material to avoid interceptions or delays.

How to Submit a Sample

Please note that it is best to submit samples to the lab early in the week to preserve the integrity of the sample and accurate processing. Due to local regulations, please label the box containing live plant material to avoid interceptions or delays.

1. If you are a new client, fill out and sign our terms and conditions form

2. Prepare your shipment using our Custom Diagnosis Sampling Guide and order form

3. Print out a copy of either*:

  Download CDFA Permit 3250
(California customers)
Download USDA APHIS PPQ
(Out-of-State Customers)
 

4. Include in your shipment package:

  • Terms & conditions form if you are a new customer
  • Custom Diagnosis order form
  • CDFA Permit 3250 (for California customers) OR USDA APHIS PPQ (for out of state customers)
  • Sample

5. Place a shipping label on your package
